How do you create a form with tick boxes which cannot be altered by the user.

Thank you for your question and welcome to the forum.

To create a form in Word:
1. Use a table structure to create your form - this will make it easier to line form elements up across your page. You can also remove the borders around the table if you don't want people who are using the form to see the borders.

2. Use the Forms toolbar to draw or insert form elements into the table (View - Toolbars - Forms). You can use the Forms toolbar to add text fields, tick boxes etc to your form.

3. When you have finished creating your form, click the Protect Form button (with the padlock icon) on the Forms toolbar. Your form will be ready to be filled in now, i.e. people filling in the form will only be able to select or type into the elements that you have drawn or inserted using the Forms toolbar.

If you wish to amend the form, you will need to click the Protect Form button again to take the protection off the document.
